Patient Forms

What to bring

Please bring your driver’s license, insurance cards, any recent diagnostic studies (X‑rays, CT, MRI and reports), and a current list of medications with dosages to your first visit.

Tip: If you’re unsure which packet to complete—or you’re being seen for both neck and back—start the forms and select the option that best fits; our team will confirm at check‑in.

Established Patients

If you haven’t been seen in over 3 years, you would be considered a New Patient.

Insurance & Payment Information

We participate in most insurance and managed‑care plans and will file claims on your behalf. If your plan requires a referral or prior authorization, please obtain it from your primary‑care provider before your visit. When an insurance carrier requests additional information, timely responses help your claim process without delays.

If your treatment follows an auto accident or other third‑party liability event, we can submit to your group health insurance; however, we do not accept third‑party liability insurance. Our billing team is happy to answer questions about coverage, billing, and special arrangements.

Co‑payments, deductibles, and any non‑covered services are due at the time of your appointment. We accept Visa, MasterCard, American Express, and Discover. For plan-specific details and a current list of accepted insurances, please contact our office.

Prescriptions & Refills

Medication requests are handled during regular office hours. For refills, contact your pharmacy at least two business days before you run out so we can review and approve safely. Narcotic prescriptions cannot be refilled outside office hours.
